Once your child has been accepted at Camp, we will be sending you some paperwork to complete. The paperwork will take only a few minutes of your time. We make the paperwork process as simple as state regulations and common sense will let us.
Please fill out and return the following forms
as soon as possible, but no later than June 1, 2008:
Administration of Medication Form*
Parents should also make it a point to contact the office of your child's physician. State Law requires us to have certain information on file. Please have your child's physician complete this form and have it sent to us as soon as possible, no later than June 1, 2008.

** It is not necessary to have a new exam if your child's most recent exam was within the last 12 months. We will accept other versions of this form if the physician's office prefers to submit their own version.
